Making a complaint or compliment

The complaints leaflet is a guide and complaints form that explains to you, what you can complain about, how you can go about complaining and what happens after you make a complaint.
The young people who attend 12-16 Foster group designed this complaint form in 2004.
If you would like a complaints form you can contact your social worker or if you do not want to do that you can contact Kevin Donovan 020 8227 5942 or Anna Muller 020 8227 3961 and they will send you a copy.
Alternatively, you can fill out the online complaints form which will be sent to a complaints officer at Children's Services.
If you don't want to fill out a complaints form to make a complaint, you can call the free phone complaints line and they will make a note of your complaint and try and sort it out for you.

Our compliments form is a form you can use to tell someone that you appreciate what they have done for you. Its totally voluntary on your behalf. The compliments form is available from your social worker, YPA or can be downloaded (pdf 456kb).
You can also request a form from the complaints manager (Her details are above) or a member of the participation and children's rights team please call 020 8227 5941. You can pay a compliment to anybody who provides a service for you, so long as they work for Children's Services.
All you have to do is complete the form, detach it from the leaflet, fold it over, seal it and pop it in the post and its free.
It may sound strange but our complaints manager also manages our compliments. When you compliment is received, our complaints manager will log the compliment forward it to the person who the compliment is for and also their manager.
