This form is to be used only for agencies referring a child/children to Children's Services for assessment as a child in need, including in need of protection. Any members of the public should contact the Child Protection Team.

Multi-Agency Core Group Monitoring Form
This form is to be used by all relevant agencies visiting a child with a Child Protection or Children in Need Plan. It helps provide a clear, consistent and accountable framework by which visits are recorded and shared with the other relevant agencies.

Common Assessment Framework
The Common Assessment Framework (CAF) is a key part of delivering frontline services that are integrated and focused around the needs of children and young people. The CAF is a standardised approach to conducting an assessment of a child's additional needs and deciding how those needs should be met. It can be used by practitioners across children's services in England.
